Levels of Service

Residents served by this plan are generally independent for personal care needs but may require assistance with bathing or daily medication administration and occasional help with activities of daily living. All Basic services are included in this plan. Three meals per day in the dining room, snacks, weekly laundry and housekeeping, 24 hour available assistance, activity programs, shuttles to Medical Appointments, and basic utilities.

Level II

Residents served by this plan receive moderate assistance with personal care and other activities of daily living. This plan is generally suitable for those needing more sporadic help throughout the day, for example: treatment issues, insulin injections, blood sugar testing, reminding and redirecting behavior, and or dressing assistance.

Level III

Residents served by this plan generally need assistance at regular, predictable, intervals throughout the day. They may need help with dressing, transfers, ambulation, and incontinence care. They may also have more complex medical regimes requiring special RN supervision. A resident on this plan may receive additional housekeeping and laundry services. This plan may also be appropriate for those needing closer supervision due to problems associated with the early stages of dementia.

Hospice Care

It is our goal that residents who choose Mill Creek Point may be allowed to age in place when ever possible. Hospice Care can be coordinated through the local Hospice and Mill Creek Point. Hospice Care can allow the resident and family to remain in familiar and comfortable surroundings aided by staff they have come to know and trust.
